Aflac Wellness Benefits and Direct Deposit Options
What are wellness benefits?
Do you get paid to see your doctor? Aflac wellness benefits are designed to provide members with a cash incentive to see your doctor on a regular basis. Regardless of what your health insurance charges for the visit, these wellness benefits reward policyholders for getting a wide range of examinations.
How do I file a claim?
The easiest way to file a claim is to call the MEA Benefits Department (888.217.9175) or to contact Will Stover at wstover@integratedlabor.com. Once the form is returned to MEA, it will be processed and submitted to Aflac. Aflac’s Claims Department tends to process all wellness benefits within 1 business day of receiving the claim. Depending on the mail, you could have your check within 3-4 business days of submitting your claim. If that is not fast enough, you can request an Aflac direct deposit form in order to have the funds deposited into your account the same day it is processed.
Here is a list of the most popular plans and their respective wellness benefits:
Group Accident:
$60.00: After 12 months of paid premiums and while coverage is in effect, Aflac will pay $60 for preventive testing. Benefits are payable for annual physical exams, mammograms, pap smears, eye examinations, immunizations, flexible sigmoidoscopies, PSA tests, ultrasounds and blood screenings. This benefit is paid per person covered under the plan, per calendar year.
Group Hospital Indemnity:
This benefit is available every calendar year, per covered person.
$100: Mammogram per calendar year, per person insured
$50: Pap exam per calendar year, per person insured
Group Critical Illness:
$200: Mammogram- available every calendar year, per covered person (not including children).
$50 Health Screening benefit- After the waiting period (30 days from your application date), a person insured may receive a maximum of $50 for any one covered health screening test per calendar year. The tests include:
• Colonoscopy• Cervical cancer screening• Breast ultrasound• Chest X-ray• PSA (blood test for prostate cancer)• Stress test on a bicycle or treadmill• Bone marrow testing• CA 15-3 (blood test for breast cancer)• CA 125 (blood test for ovarian cancer)• CEA (blood test for colon cancer)• Flexible sigmoidoscopy• Hemocult stool analysis• Serum protein electrophoresis (blood test for myeloma)• Thermography• Fasting blood glucose test• Serum cholesterol test to determine level of HDL and LDL
This benefit is payable for the covered employee and spouse but not dependent children.
Cancer Plan:
These are preventive benefits and a cancer diagnosis is not required for these benefits to pay out. Benefits are payable once per calendar year, per person covered under your plan.
$100: Mammogram per calendar year, per person insured
$30: Pap exam per calendar year, per person insured
$75: for any one of the following:
•Biopsy• Flexible sigmoidoscopy• Hemocult stool specimen• Chest X-ray• CEA (blood test for colon cancer)•CA 125 (blood test for ovarian cancer)• PSA (blood test for prostate cancer) •Thermography
• Colonoscopy or virtual colonoscopy.
Note: Not all cancer plans have the same wellness benefits.
Intensive Care Plan:
$150: Mammogram per calendar year, per person insured
