Have you submitted your Aflac Wellness claims?
Over the last 3 months, Aflac has paid city employee policyholders over $47,000 in wellness benefits. Many people forget that they have this benefit, so we want to ensure our members receive all their eligible benefits. Aflac wellness benefits are designed to provide policyholders with a cash incentive to see your doctor.
Here are common exams eligible for wellness benefits:
- Blood test
- Mammogram
- PAP
- Physical
- Eye exam
- Colonoscopy
- PSA
- Immunization
- Ultrasound
For a complete list and eligibility, click here. If you see something that applies, please email us with the following information, and we will send you any eligible claim forms:
- Name and Date of Birth of patient
- Date and Type of exam
- Location of exam (This can be generic, e.g. “Sharp Grossmont”)
As long as I can remember I’ve had AFLAC cancer coverage through work. My plan pays me to have my early physical if certain tests are done under my coverage.
